Examiner Guidance: Approach to History in Acute Infantile Heart Failure

Always distinguish acute myocardial dysfunction (myocarditis / DCM) from left-to-right shunt. In acute myocarditis, symptom onset is rapid (days), often preceded by a viral prodrome (enterovirus, adenovirus), with profound cardiogenic shock features (poor pulses, cool peripheries, oliguria) rather than a chronic history of recurrent pneumonia and long-standing failure to thrive.

  • Fast Breathing & Respiratory Distress:
    • Began 5 days ago, acute in onset, rapidly progressive.
    • Fast breathing present persistently, even while sleeping.
